Sending a client takes about five minutes.
There’s no long form to fight with. Send us the essentials and our intake team handles the rest — screening, matching, funding, and scheduling.
What to include
Everything we need, nothing we don’t.
A short note is enough to start. If something on this list is missing, we’ll follow up rather than send the referral back.
- Client name, date of birth, phone number, and email
- Reason for referral and how long it's been going on
- Current treatment, medications, and any past programs
- Level of urgency, and any risk you're aware of
- Funding source, if you know it (benefits, employer, workers' compensation)
- Your name, role, and the best way to reach you
Please confirm your client knows the referral is being sent and consents to being contacted.

What happens next
You’ll know where it landed.
Acknowledgement
You get confirmation that the referral arrived, within one business day.
Screening call
An intake clinician calls your client, usually within three business days, to understand needs and confirm funding.
Recommended pathway
We recommend therapy, a structured program, or medically assisted therapy — and book the first appointment.
Update back to you
With your client’s consent, we send a short summary of the plan and agree on how often you’d like progress notes.
Urgent situations
We are not a crisis service.
For time-sensitive but non-emergency referrals, call intake and say it’s urgent. We keep a small number of expedited screening slots each week.
If your client is at immediate risk, direct them to emergency services or the nearest emergency department. Once they are stable, we can pick up ongoing outpatient care.
