Physician resources

The clinical detail you need before you refer.

What each program involves, who it suits, what we report back, and how medication and collaborative care are handled.

Criteria

What we look for at intake.

We assess suitability rather than screen people out. If a client doesn’t meet criteria for a program, we recommend a different level of care instead of closing the file.

referrers hero
  • Adults 18+ able to participate safely in outpatient care
  • A primary mental health or substance use concern
  • Willingness to attend scheduled sessions in-person or by video
  • Funding confirmed at intake — benefits, workplace program, or self-pay

Case discussion welcome

If a presentation is complex, you can speak with a clinician before referring. Call intake and ask for a clinical consultation — usually arranged within two business days.

What you'll hear back

Communication you can plan around.

Receipt confirmation

Initial plan summary

After the screening call: recommended level of care, start date, and goals.

Progress notes

At intervals you set — mid-program, at discharge, or on request.

Medication coordination

Where a physician on our team is involved, prescribing decisions are shared with you rather than made around you.
